Job Description Job Description San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. St. Paul's is offering a $1,500 signing bonus for PACE LVNs! Conditions apply. Schedule: Wednesday - Sunday 8:00 am to 4:30 pm Site: This position travels to participant's home and involves extensive use of reliable transportation throughout San Diego County for home visits. Job Summary: Under the direction of the In-Home Services Director, provides skilled nursing services (medication administration, wound care, etc.) to the participants of St. Paul’s PACE in their homes. Maintains the participants’ medical record with thorough documentation. Communicates participants’ changes and progress to appropriate staff. Qualifications and Requirements: Education - Graduate of a school of Licensed Vocational nursing. Current California Licensed Vocational Nurse license, CPR and first aid certification required. Shall be at least 18 years of age. Experience – A minimum of one year’s experience in working with the frail or elderly population required. Required qualifications:

• Graduate of a school of Licensed Vocational nursing
