Plays a crucial role in conducting ongoing patient assessments, documenting data in medical records, and coordinating multiple appointments to ensure comprehensive communication across disciplines within clinical programs. Navigates the healthcare continuum for patients and families, engaging multidisciplinary resources to optimize outcomes, and guide both patients and internal healthcare team members on clinical care management and transitions. Additionally assists in implementing clinical specific continuing education, ensuring compliance with best practices and professional standards, while adhering to established policies, procedures, and regulatory requirements specific to clinical care. We are seeking a highly organized and detail oriented Clinical Coordinator II to support patient identification and onboarding for clinical trial participation within the BMTCT service. This role is responsible for reviewing patient records, diagnostic imaging, and other testing to determine potential eligibility for clinical trial protocols. The Clinical Coordinator II will work closely with physicians, principal investigators, and BMTCT staff to facilitate prioritization and eligibility decisions. As the first point of contact for referring providers, patients, and families, this position coordinates travel and logistics, provides orientation to St. Jude Children’s Research Hospital, and ensures a smooth transition into evaluation and treatment. The coordinator serves as a key liaison across internal clinical services and external partners to ensure timely communication, coordinated care, and an exceptional patient experience. This position is eligible for hybrid work flexibility.
Responsibilities
Coordinate patient eligibility review by gathering medical records, diagnostic imaging, tissue slides, and HLA typing; submit materials for service review and relay results to physicians/PI for eligibility decisions.
Serve as primary liaison between BMTCT and referring providers to ensure timely communication, appropriate travel timing, and smooth transition of patients into St. Jude evaluation and care.
Support patients and families by coordinating travel, providing orientation to St. Jude services and amenities, and connecting them with housing, social work, and other support resources; assist with obtaining passports/visas when needed.
Collaborate across clinical services by attending relevant meetings and facilitating communication between BMTCT and other departments involved in patient care.
Responsible for on-going patient assessment, data collection and documentation in the medical record
Assist with coordinating multiple appointments, utilizing the scheduling systems to ensure thorough communication across the multiple disciplines.
Monitor and ensure effective navigation of the healthcare continuum for patients and their families at St. Jude.
Assist in implementing nursing continuing education in compliance with best practices and education with American Nurses Credentialing Center (ANCC) and Tennessee Nurses Association (TNA) guidelines for continuing education and with the Nursing Education department’s Continuing Education Policy.
Engage the appropriate resources within the multidisciplinary team to achieve optimal results for the patient, family, and caregivers;/ clinical team.
Ensure interdisciplinary plan of care oversight and coordination of disciplines to optimize outcomes.
Guide patients and family members on all aspects of care and transitions (both ambulatory and inpatient).
Educate internal members of the health care team on care management and managed care concepts.
Perform all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards.
Perform other duties as assigned to meet the goals and objectives of the department and institution.
Maintains regular and predictable attendance.
Requirements
Bachelor’s Degree required.
Minimum requirement: 4+ years of relevant experience.
Solid understanding of the patient care system at St. Jude.
Must possess a current Tennessee State Board of Nursing license if primary residence is Tennessee or a Nurse Licensure non-Compact state.
Must possess a current State Board of Nursing license in the state of primary residence if the state is a Nurse Licensure Compact state.
Nice-to-haves
Some experience using specialized equipment and patient education materials for the care of the pediatric oncology and hematology patients, preferred.
Some experience recording and maintaining data on educational activities, preferred.
Working knowledge of infection control policies and universal precautions.
Strong understanding of oncology and/or bone marrow transplant, preferred.
